Did you know the Health Equity Alliance of Rock County creates a Community Health Improvement Plan (CHIP) every 3-years to find out what they will focus on? These priorities guide where resources and funding are directed in our community to make a greater impact throughout the county. To make this plan, they created a Community Health Survey where they aim to get 2,000 responses. If you want to provide input, the survey is your way to advocate and vote for resource allocation in Rock County.
